Patient Info

If you are a new patient, you can save time by filling out your health history in advance! It is simple, just follow the steps below.

  • Provide us with your email address when you make your appointment.
  • We will invite you to join our Patient Portal and you can fill out your health history in advance.
  • You will be able to receive secure test results and message (securely) with your provider over the Patient Portal.

Arrival Time

We value the one-on-one time we get with each patient. As such, we strive to see each patient at their scheduled appointment time. We ask that you do your part in keeping us on schedule by arriving at least 15 minutes prior to your appointment. This gives our front-end staff the necessary time to review any insurance or paperwork questions we may have.

What to Bring

Please bring the following with you to each appointment:

  • Identification
  • Insurance card
  • List of any medications you are currently taking
  • Paperwork
  • Co-pay

Laboratory Services

As a patient in our practice, there will be times when it will be necessary to have your blood drawn. Please note that it can take up to a week for our office to receive your test results. Within 24 hours of our providers receiving your results we will contact to discuss your results.

Office Hours

Our office is open Monday through Friday from 8 a.m. to 5 p.m. We take lunch from noon to 1 p.m. During this time our phone lines are off.

Cancellation Policy

While we understand that life happens, we ask for 48 hours advance notice if you cannot make your scheduled appointment. We reserve the right to charge patients $50 for not cancelling in a proper amount of time or for no shows.

Prescription Refills

If you are in need of a refill, please call your pharmacy. They will facilitate the necessary steps in getting your medication refilled. Please note, that we ask for at least 24 hours notice when refilling medications. Narcotics will NOT be refilled without an office visit.

Medical Records

If you are new to our office we will work with you to have your previous medical records sent to our office.

If you are leaving our practice (we hope you won’t) we will help you transfer your medical records to your new office.
